In order to register your child at the Block island School, we must have a copy of the following:

• Birth Certificate

• Immunization Records

• Physical Examination Form/Documents (see note below)

• Registration Form

• Accident Form

• Release Form

Note: If your child was not previously enrolled within the Rhode Island School System, your child must have an up-to-date physical exam (within the last 6 months).  Without the above information, we cannot accept your child into the Block Island School.
